The router is a separate unit. The 3G networking piece is provided by a USB data thingy provided by Verizon or Sprint. Both carriers provide nationwide service. Basically it works anywhere there's cell service. In most areas (including all major metro areas) the connection is fast enough to support 64k or 128k streaming. Out in the sticks, the connection may be slower — but it's always fast enough for 24k AAC+, which still sounds pretty decent.

My preferred in-car listening setup is a 3G WiFi router (with a Verizon data card) + an iPod Touch. That way I can listen to RP via the iPod, and connect to the web on my laptop (or multiple laptops/iPods if I have passengers), all with a single data account. Verizon's network is also faster & more reliable (at least in my opinion) than AT&T's.
The best 3G WiFi router I've tested is the Cradlepoint CTR-500 ($179 from Amazon). That coupled with a USB data card from Verizon, Sprint, or AT&T ($50 - $60/mo) & an iPod Touch is the ultimate in-car listening setup.
Of course, if you want to listen away from your car & home, an iPhone or a multimedia Blackberry is the way to go. The iPhone is by far the better performer for web radio, but the Blackberry Curve & Bold are OK also (I hated the Blackberry Storm that I tried).
The Blackberrys also do not come equipped to play web audio. I needed to buy a app called BerryTunes ($20). On the iPhone there are plenty of free apps (including our own Radio Paradise app) that will play RP. BerryTunes plays audio, but does not display song titles or other info.
Another note: look for a new, improved mobile-device version of the RP website debuting soon (this weekend? next week? something like that...). Stay tuned.
